Audience Response System Loan Agreement

Use this form to reserve the TurningPoint Audience Response System. Please allow a minimum of 72 hours for scheduling. If your timeframe is shorter, please e-mail Lara Westphal immediately after submitting this form, and we will attempt to accomodate you.

General Information




You will attach the receiver to the computer you are using to give your presentation. The computer must have TurningPoint installed. There is a computer in each of the medical student classrooms with TurningPoint installed that you can use for your presentation, but if you are presenting in another location, you will need to provide a computer or laptop with TurningPoint installed.

The Audience Response System equipment is available for pick-up at MDC 2535. Please indicate your first and second choices of when you would like to pick-up the equipment below. Your pick-up time will be finalized during confirmation. A member of CSI2 will be available in either MDC 2534 or MDC 2535 to assist you with checking out the equipment at your scheduled time.

Please note that your PowerPoint presentation must be specially designed and prepared to work with the audience response system. Information on using TurningPoint and software downloads are available at TurningPoint 2008 Student and Instructor Guides. For training on how to incorporate TurningPoint into your PowerPoint presentation, contact Chris Hoelle 974-6288. It is highly recommended that you attend a training session prior to reserving the audience response system.

We can help you design highly effective and innovative educational activities that maximize the features of the audience response system while meeting your educational objectives. We also would like to help you to ensure that the technology runs smoothly during your event by scheduling an optional test run of the system with you or helping with set-up. Please indicate if you would like someone from CSI2 to contact you to discuss additional instructional design or technology support options.


Fees are generally waived for College of Medicine Undergraduate Medical Education lectures and events. Your confirmation will specify the total fees. If any fees are due, your billing account number will be requested during confirmation.

Terms of Loan

The Audience Response System is inspected before each project. The person requesting the equipment assumes responsibility for its condition. Any damage of loss of equipment will be billed to the signer's department. Agreed values are $60 for each RF card and $120 for each receiver. Your electronic signature on this form indicates acceptance of these terms.
